Take advantage of 24/7 access to healthcare, right at your fingertips. With MyUnityPoint, powered by MyChart, you have the best access at your fingertips.
Access your MyUnityPoint account to:
- Communicate with your doctor. Get answers to your medical questions from the comfort of your own home.
- Access your test results. No more waiting for a phone call or letter – view your results and your doctor’s comments within days.
- Request prescription refills. Send a refill request for any of your refillable medications.
- Manage your appointments. Schedule your next appointment, or view details of your past and upcoming appointments.
To access your MyUnityPoint account:
- Log in to your MyUnityPoint account.
- Don’t have a MyUnityPoint account? You will need to create an account at chart.myunitypoint.org to request access.
- You do not have to be a UnityPoint Health patient to create an account.
- To set up Proxy Access to your MyUnityPoint on behalf of a family member:
- Once logged in, select “Share My Record” 01 the drop-down menu.
- Select “Friends and Family Access” in the “Share with People” section.
- Select the applicable question for your request under the “Who Can See My Record” section.
- Are you a parent or guardian?
- Do you need access to an incapacitated patient?
- Do you know someone who should not have access?
- Want another adult to have access to your record?
- Fill out the required fields. If you are requesting proxy access for a minor, the health information available to you will vary based on the patient’s age. Please refer to the Proxy Access and Document Requirements on the back for more information.
- Click “Submit Request”
Please note: There is a 3–5-day turnaround time for proxy access to be granted. If you are requesting proxy access for an incapacitated patient, it may take up to 4 weeks.

Insurance – Important questions to ask your insurance company.
- Do you have OB/maternity coverage on your policy?
- Is your medical provider in-network with your insurance?
- Deductible amount?
- Deductible amount met for the year to date?
- What month does your deductible start?
- What percent does your insurance pay after the deductible is met?
- Are OB labs and ultrasounds subject to copay OR deductible and co-insurance?
- What is your max out-of-pocket?
- Does your max out-of-pocket include the deductible?
