Description

Openings in Colchester, LaHarpe, Hamilton and Carthage.

This is a clinical practitioner who participates in providing patient care. He/she is able to implement and evaluate patient care specific to the age of the patient population served from newborn to the elderly. He/she ensures that quality care is provided in an efficient and safe manner, consistent with the unit's standards of care. He/she demonstrated performance consistent with the mission, philosophy and goals of the unit and organization; remains flexible to changing system; is expected to demonstrate quality and effectiveness in work habits and clinical practice; and treats staff, physicians, patients and families with consideration and respect. Acts as a patient advocate at all times. To assist the primary provider in all professional and clinical procedures. Clinical procedures shall include phlebotomy, performing EKG's, urinalysis, injections, and taking and recording patient vital signs.

Description

 

Position is part time at our Women and Family Clinic in Carthage.  Monday - Thursday 5 p.m. to 8 p.m. and Saturday 8 a.m. to 2 p.m.

Performs clerical duties, which include intake and check out of patients.  Provides patient appointments based on protocol and phone assessment of patient condition.  Collects co-pay and deductible amounts from patients and tracks patient financial and productivity information. 

Primary Job Responsibilities:

  1. At each visit will verify the patient’s date of birth, phone number, address, insurance, and any other necessary information.
  2. Records necessary demographic data changes in the chart and computer.
  3. Maintains appointment schedule with distribution to nursing staff and providers as requested.
  4. Prepares the charts for the next day appointments making sure chart is neat, up to date and non-tattered.
  5. Files all charts.
  6. Collect co-pays and patient balances at the time of service.  Have self pay patients fill out forms and payment contracts as needed.
  7. Answers the telephone by the third ring.
  8. Takes precise messages by getting the patient’s full name and spelling, date of birth, complete description of phone call, initials, and time of call.
  9. Responsible for incoming and outgoing mail.

Description

The Certified Nurse Assistant is responsible for providing personal care to the residents. Personal care includes assistance with meals, dressing, movement, and bathing. The CNA follows established facility policies and procedures while providing care and coordinates resident care with other facility personnel and families.

Description

Performs a variety of general cleaning tasks to maintain patient/resident rooms, offices, hallways and other assigned areas of the hospital.

Primary Job Responsibilities:

  1. Dusts furniture, cleans and sanitizes doors, walls, windows, window sills, and floors daily.
  2. Cleans and disinfects all fixtures, floors, mirrors, windows, doors and walls of bathrooms and showers.
  3. Uses and/or wears the proper safety equipment and uses universal precautions at all times.
  4. Corrects unsafe conditions when found.
  5. Notes needed repairs and communicates them to appropriate people.
  6. Gathers and disposes of trash and waste materials in accordance with specified procedures regarding bags, containers, biohazard and infection control practices.
  7. Vacuums or wet/dry mops rooms, offices, halls. Stairways and public areas as assigned.
  8. Cleans all departments and assigned areas daily which include visitor areas, waiting rooms, offices and other support areas.

Nurse Care Coordinator

Description

The Care Coordinator is responsible for functioning in an expanded role to facilitate care through interdisciplinary collaboration and care coordination. Responsible for communicating with patients and caregivers to ensure improved care transitions, enhancing patient to provider communication to identify opportunities for care continuity, encouraging the patient’s self-management skills and understanding the healthcare system to guide and empower patients to participate in their own care plan.

  1. Help establish the Chronic Care Management/Patient-Centered Medical Home Programs.
  2. Continually assess and provide suggestions to improve Care Plans.
  3. Document all patient care interactions in Electronic Health Record.
  4. Must have superb time management skills.
  5. Continually assess and improve relationships with all healthcare resources.
  6. Must have superb assessment skills and be able to relay those to the healthcare provider in a professional manner.
  7. Be able to float from the hospital and the clinic to help with the coordination of care upon discharge.

Description

Responsible for processing claims of all third party payers and private pay patients, both inpatient and outpatient.  Handles correspondence to and from insurance companies regarding submitted claims.  Makes appropriate contacts, by telephone or correspondence, with patients or insureds regarding needed information to accurately bill patient's claim.  Prepares vouchers from Medicare, Medicaid, and TriCare (Champus) for posting to patient accounts.  Must be available to the public, walk-ins and telephone calls, for any inquiries.
